Allergic Reactions to Natto—Context and Interpretation

and is intended to summarize its significance and place it in proper biochemical and clinical context. The original paper is a short and technical case report, and without additional explanation, its importance can easily be misunderstood.

1. Background: Soy Allergy and Soy Allergens

Soy allergy is caused by immune reactions to specific intact soy proteins, not to soy-derived substances in general. The major recognized soybean allergens include

  • Gly m Bd 60K
  • Gly m Bd 30K (approximately 34 kDa)
  • Gly m Bd 28K (approximately 26 kDa)

These are large, structurally stable proteins that retain their allergenicity through cooking and digestion. For this reason, unfermented soy products are well known and well documented as common food allergens.

2. What Fermentation Changes in Natto

Natto is produced by fermenting soybeans with Bacillus subtilis natto. During fermentation, this bacterium produces powerful proteolytic enzymes that:

  • Break large soy proteins into much smaller peptides
  • Disrupt or destroy the original three-dimensional protein structures
  • Eliminate or alter the IgE-binding epitopes responsible for allergic reactions

In practical terms, this means that the major soy allergens are no longer present in their original, allergenic form. Although soy-derived material remains, its biochemical identity is substantially transformed.

This mechanism is well understood in food science and helps explain why fermentation often reduces allergenicity in foods.

3. Why Allergic Reactions to Natto Are Extremely Rare

Despite the high prevalence of soy allergy, allergic reactions to natto have almost never been reported. This strongly supports the view that fermentation substantially reduces allergenic potential.

The paper in question explicitly notes this rarity. It exists because the authors encountered an exceptional case, not because natto is generally allergenic.

4. Understanding the Case Report in the Paper

The reported patient showed an unusual pattern:

  • Sensitivity to natto
  • No sensitivity to unfermented soybean

Such cases are biologically plausible but exceedingly rare, which is why the authors considered it worthy of publication.

Nattokinase supplements differ significantly from whole natto:

  • Nattokinase is an isolated enzyme, not a whole fermented food
  • It is typically highly purified
  • Many preparations are tested to confirm the absence of detectable soy proteins
  • Some are produced using non-soy fermentation substrates

As a result, nattokinase supplements generally contain even less potentially allergenic material than natto itself.

6. Why Labels Still Warn About Soy

Soy warnings on natto or nattokinase products usually exist for regulatory and legal reasons rather than biochemical ones:

  • The starting material may be soybeans
  • Trace residues cannot always be excluded at an absolute level
  • Food and supplement labelling rules prioritise caution over biochemical nuance

These warnings do not imply that intact soy allergens are present, only that soy was involved somewhere in the production process.
